TIMI Risk Score Calculator (UA/NSTEMI)

The TIMI risk score for unstable angina / non-ST-elevation MI sums seven equally-weighted clinical predictors (0–7) to estimate the 14-day risk of death, new or recurrent myocardial infarction, or severe recurrent ischaemia needing urgent revascularisation.

Quick answer

The TIMI risk score for UA/NSTEMI adds one point each for seven predictors β€” age 65 or over, three or more CAD risk factors, known coronary stenosis β‰₯50%, aspirin in the past 7 days, two or more anginal episodes in 24 hours, ST deviation β‰₯0.5 mm, and elevated cardiac markers. The total (0–7) maps to a 14-day risk of death, MI, or urgent revascularisation from 4.7% to 40.9%.

TIMI risk predictors (1 point each)

0/ 7

Low risk (0–2) Β· 14-day event rate 4.7%

The TIMI risk score is 0/7: low risk (0–2). In the Antman 2000 derivation, this corresponds to a 14-day rate of 4.7% for the composite of death, new or recurrent MI, or severe recurrent ischaemia requiring urgent revascularisation.

This is the UA/NSTEMI version (a separate TIMI score exists for STEMI). It is a prognostic aid, used with the full clinical picture, troponin and ECG β€” not a substitute for clinician assessment.

How to use the TIMI (UA/NSTEMI) calculator

  1. 1Tick the seven predictors. Tick each predictor that is present: age β‰₯65, β‰₯3 CAD risk factors, known coronary stenosis β‰₯50%, aspirin use in the last 7 days, β‰₯2 anginal episodes in 24 hours, ST-segment deviation β‰₯0.5 mm on the admission ECG, and elevated cardiac biomarkers.
  2. 2Read the score. The tool sums the points (0–7).
  3. 3See the 14-day event rate. The tool shows the Antman 2000 14-day composite event rate (death / MI / urgent revascularisation) for that score, and a low / intermediate / high band, to interpret with a clinician.

Frequently asked questions

What does the TIMI score predict?βŒ„

In UA/NSTEMI it predicts the 14-day risk of the composite of all-cause death, new or recurrent myocardial infarction, or severe recurrent ischaemia requiring urgent revascularisation. The rate rises from 4.7% at a score of 0–1 to 40.9% at 6–7.

Is this the same as the TIMI score for STEMI?βŒ„

No. This is the TIMI score for unstable angina / NSTEMI (Antman 2000). There is a separate, differently-weighted TIMI score for ST-elevation MI (STEMI). Use the one that matches the presentation.

Does aspirin use really increase the TIMI score?βŒ„

Yes β€” recent aspirin use is one of the seven predictors. People already taking aspirin who still develop an acute coronary syndrome tend to have higher risk, which is why it scores a point in the original model.

Is my data stored?βŒ„

No. The calculator runs entirely in your browser; nothing you enter is sent to a server.
