Padua Prediction Score Calculator (VTE Risk)

The Padua Prediction Score estimates the risk of venous thromboembolism (VTE) in hospitalised medical patients from eleven clinical factors, to decide whether thromboprophylaxis is warranted. A score of 4 or more indicates high risk.

Quick answer

The Padua Prediction Score adds eleven weighted risk factors to estimate venous thromboembolism (VTE) risk in hospitalised medical patients. A total of 4 or more indicates high risk, where thromboprophylaxis is usually considered; under 4 is low risk.

Low risk of VTE (< 4)

The Padua Prediction Score is 0: low risk of vte (< 4). A score of 4 or more identifies hospitalised medical patients at high risk of venous thromboembolism, where pharmacological thromboprophylaxis is usually considered if there is no contraindication.

The score does not assess bleeding risk; the prophylaxis decision weighs both and is your clinician's. For surgical patients the Caprini model is generally used instead.

How to use the Padua VTE calculator

  1. 1Tick the high-weight factors. Tick the 3-point factors present: active cancer, previous VTE, reduced mobility, or known thrombophilia.
  2. 2Tick the remaining factors. Tick recent (under 1 month) trauma or surgery (2 points), and the 1-point factors: age 70 or over, heart and/or respiratory failure, acute MI or stroke, acute infection or rheumatologic disorder, obesity, and hormonal treatment.
  3. 3Read the score. The tool totals the points. A score of 4 or more is high risk for VTE, prompting consideration of thromboprophylaxis alongside bleeding risk and clinical judgement.

Frequently asked questions

What Padua score means high VTE risk?βŒ„

A Padua Prediction Score of 4 or more indicates high risk of venous thromboembolism in hospitalised medical patients, where pharmacological thromboprophylaxis is usually considered if there is no contraindication.

Is the Padua score for surgical patients?βŒ„

No β€” it was derived and validated in hospitalised medical (non-surgical) patients. Surgical VTE risk is usually assessed with the Caprini model instead.

Does a high Padua score mean I will definitely get a clot?βŒ„

No. It identifies a group at higher risk, not a certainty. The decision to use thromboprophylaxis also weighs bleeding risk and is made by a clinician.
