Wells Score for DVT Calculator

The Wells score for deep vein thrombosis (DVT) estimates the pre-test probability of a leg DVT from nine clinical criteria, helping decide whether a D-dimer or ultrasound is the right next test.

Quick answer

The Wells score for DVT adds nine clinical findings (and subtracts 2 if another diagnosis is at least as likely) to estimate the probability of a leg deep vein thrombosis. A score of 2 or more is "DVT likely"; under 2 is "DVT unlikely", where a negative D-dimer can rule it out.

0

DVT unlikely

A modified Wells DVT score of 0 β‡’ DVT unlikely. In the dichotomised rule, a score β‰₯ 2 is β€œDVT likely” (proceed to ultrasound), and < 2 is β€œDVT unlikely” (a negative high-sensitivity D-dimer can safely exclude DVT). Your clinician chooses the pathway.

How to use the Wells DVT calculator

  1. 1Tick the clinical criteria. Tick each Wells DVT criterion present, such as active cancer, leg swelling, calf swelling over 3 cm, and previously documented DVT.
  2. 2Account for alternatives. Tick "alternative diagnosis at least as likely" to subtract 2 points if another cause is equally or more probable.
  3. 3Read likely vs unlikely. A total of 2 or more is "DVT likely" (ultrasound); under 2 is "DVT unlikely" (a negative high-sensitivity D-dimer can exclude DVT).

Frequently asked questions

What Wells DVT score means DVT is likely?βŒ„

In the dichotomised rule, a score of 2 or more means DVT is "likely" and usually leads to a compression ultrasound. Under 2 is "unlikely".

Can the Wells score rule out a DVT on its own?βŒ„

Not by itself. A low Wells score combined with a negative high-sensitivity D-dimer can safely exclude DVT; otherwise imaging is needed. A clinician decides.

Is this the modified Wells score?βŒ„

Yes β€” it includes the "previously documented DVT" point and the βˆ’2 for an equally likely alternative diagnosis, as in the 2003 modified rule.
