How to use the SOFA calculator

  1. 1Enter the organ-system values. Enter the PaO₂/FiO₂ ratio (and whether on ventilatory support), platelet count, bilirubin, Glasgow Coma Scale and creatinine (or urine output).
  2. 2Choose the cardiovascular state. Select the cardiovascular category from the published options: MAP ≥70 with no vasopressors (0) up to high-dose vasopressors (4).
  3. 3Read the total SOFA. The tool sums the six sub-scores (0–24) and shows the total. Interpretation — and especially a 2-point rise from baseline for sepsis — belongs to a clinician.

What is a normal SOFA score?

A SOFA of 0 means no measured organ dysfunction across the six systems. In Sepsis-3, an acute increase in total SOFA of 2 points or more (in a patient with suspected infection) is used to define sepsis-related organ dysfunction.

Does a high SOFA score predict death?

In the original cohorts, higher and rising SOFA scores were associated with higher ICU mortality, but the exact risk depends on the population and the patient’s baseline. This tool reports the total and a general band, not a mortality percentage, because that varies by setting.

What is the difference between SOFA and qSOFA?

qSOFA is a 3-item bedside screen (respiratory rate, systolic blood pressure, altered mentation) used outside the ICU to flag patients who may need closer assessment. The full SOFA score uses labs and organ-support data and grades dysfunction in six systems.
