How to use the MELD 3.0 calculator

- 2Read the MELD 3.0 score. The tool applies the published bounds (creatinine 1.0β3.0, bilirubin/INR β₯ 1.0, sodium 125β137, albumin 1.5β3.5) and the MELD 3.0 equation, returning a score of 6β40.
- 3Interpret with a transplant team. Higher scores indicate higher 90-day mortality and higher transplant priority. Listing decisions belong to a transplant team.

What is the difference between MELD, MELD-Na and MELD 3.0?β
The original MELD used creatinine, bilirubin and INR. MELD-Na added sodium. MELD 3.0 (2021) further added albumin and a sex adjustment and updated the coefficients, and it has been used for U.S. allocation since July 2023.
What MELD 3.0 score is high?β
The scale runs 6 to 40. Higher scores indicate greater 90-day mortality risk; scores in the 30s are very high. A transplant team interprets the number in full context.
Why are the lab values capped?β
The published MELD 3.0 rule bounds each value (for example creatinine to 1.0β3.0 mg/dL) so that extreme readings do not distort the score. This calculator applies those bounds exactly.
