How to use the MELD 3.0 calculator

  1. 1Enter the lab values. Enter bilirubin, INR, creatinine, sodium and albumin, and select sex. Tick the dialysis box if the patient had dialysis at least twice in the last week.
  2. 2Read the MELD 3.0 score. The tool applies the published bounds (creatinine 1.0–3.0, bilirubin/INR β‰₯ 1.0, sodium 125–137, albumin 1.5–3.5) and the MELD 3.0 equation, returning a score of 6–40.
  3. 3Interpret with a transplant team. Higher scores indicate higher 90-day mortality and higher transplant priority. Listing decisions belong to a transplant team.

What is the difference between MELD, MELD-Na and MELD 3.0?βŒ„

The original MELD used creatinine, bilirubin and INR. MELD-Na added sodium. MELD 3.0 (2021) further added albumin and a sex adjustment and updated the coefficients, and it has been used for U.S. allocation since July 2023.

What MELD 3.0 score is high?βŒ„

The scale runs 6 to 40. Higher scores indicate greater 90-day mortality risk; scores in the 30s are very high. A transplant team interprets the number in full context.

Why are the lab values capped?βŒ„

The published MELD 3.0 rule bounds each value (for example creatinine to 1.0–3.0 mg/dL) so that extreme readings do not distort the score. This calculator applies those bounds exactly.
