GAD-7 Anxiety Screening Calculator

The GAD-7 is a seven-item generalised-anxiety questionnaire scored from 0 to 21. Each item is rated 0 (not at all) to 3 (nearly every day) over the last two weeks. Totals of 5, 10 and 15 mark the thresholds for mild, moderate and severe anxiety, with 10 or more a common referral cut-point.

Quick answer

The GAD-7 is a seven-item generalised-anxiety questionnaire. Each item is rated 0 (not at all) to 3 (nearly every day) over the last two weeks, giving a total of 0 to 21. Scores of 5, 10 and 15 are the thresholds for mild, moderate and severe anxiety, and a score of 10 or more is a common cut-point for further assessment.

Over the last 2 weeks, how often have you been bothered by…

0/ 21

Minimal anxiety (0–4)

The GAD-7 total is 0/21: minimal anxiety (0–4).

A score of 10 or more is a common cut-point for further assessment. The GAD-7 supports β€” but does not replace β€” a clinical assessment and diagnosis.

How to use the GAD-7 calculator

  1. 1Answer each of the seven items. For each of the seven problems over the last two weeks, choose 0 (not at all), 1 (several days), 2 (more than half the days) or 3 (nearly every day).
  2. 2Read the total and band. The tool adds the seven items (0–21) and shows the severity band: minimal, mild, moderate or severe.
  3. 3Interpret with a clinician. A score of 10 or more is commonly used as a threshold for further assessment of generalised anxiety. The GAD-7 supports, but does not replace, a clinical assessment.

Frequently asked questions

What GAD-7 score suggests anxiety?βŒ„

A score of 10 or more is a recommended cut-point for probable generalised anxiety disorder and for further assessment, with bands at 5, 10 and 15 marking mild, moderate and severe anxiety. A clinician makes the diagnosis.

Does the GAD-7 only screen for generalised anxiety?βŒ„

It was developed for generalised anxiety disorder, but it also performs reasonably as a screen for panic, social anxiety and post-traumatic stress disorder. A clinician decides which condition the symptoms fit.

Can the GAD-7 be used with the PHQ-9?βŒ„

Yes. The GAD-7 and PHQ-9 share the same response format and are often used together because anxiety and depression frequently co-occur. Each is scored separately.

Is my data stored?βŒ„

No. The calculator runs entirely in your browser; nothing you enter is sent to a server.
