GP vs Specialist: When to See a Specialist Online

One of the most common questions we receive is whether to book a GP or a specialist. The answer depends on your specific concern, whether a diagnosis already exists, and whether you need assessment, treatment, or a second opinion. This guide explains the decision clearly.

What does a GP do?

A General Practitioner (GP) is a doctor trained to assess, diagnose, and manage a broad range of conditions across all body systems and all age groups. A GP is your primary care doctor β€” the first point of contact for new symptoms, ongoing condition management, preventive care, and coordinating specialist referrals.

GPs handle the vast majority of health concerns without specialist involvement. They can prescribe medications, order investigations, interpret results, issue sick notes and referral letters, and manage most chronic conditions including diabetes, hypertension, and asthma.

What does a specialist do?

A specialist is a doctor who has completed additional training in a specific body system or disease area after qualifying as a GP. Cardiologists focus on the heart, dermatologists on skin, gastroenterologists on the digestive system, and so on. Specialists provide deeper expertise for complex, unusual, or refractory conditions.

On GeraClinic, you can book with specialists directly β€” without a GP referral β€” for most concerns. All specialists are board-certified in their field with credentials verified before joining the platform.

Chest pain and heart symptoms

Cardiac symptoms warrant specialist assessment. A GP may run an initial ECG but cardiac interpretation and management is cardiologist territory.

Irregular mole or skin lesion

Dermatologists have specialist training in dermoscopy and skin cancer recognition. Direct booking is faster and more accurate for pigmented lesions.

Anxiety or depression

You can book directly with a psychologist, therapist, or psychiatrist for evidence-based psychological therapy or medication management.

Joint pain or back pain

A GP first rules out medical causes (inflammatory arthritis, infection) and provides physiotherapy referral. Orthopaedic surgeon needed if surgery is being considered.

Blood sugar issues

Initial diabetes management is GP-level. An endocrinologist is needed for complex insulin regimens, type 1 diabetes, or other hormonal disorders.

Digestive symptoms

A GP manages most common digestive complaints. Gastroenterologist needed for endoscopy, IBD, persistent unexplained symptoms, or abnormal imaging.

Children's health

Paediatric consultation can be booked directly for ongoing child health concerns, developmental reviews, and conditions benefiting from specialist paediatric knowledge.

Pregnancy and women's health

Many gynaecological concerns can be booked directly β€” contraception, menstrual problems, pelvic pain, cervical smear concerns, and menopause.

Sports or exercise injury

Sports medicine specialists are the appropriate first point of contact for most musculoskeletal sports injuries. They can prescribe, order imaging, and manage rehabilitation.

Fertility concerns

After 12 months trying to conceive (6 months if over 35), a direct fertility specialist consultation provides faster access to appropriate investigation.

When a GP is the right first step

Book a GP first when:

  • β€’You have a new symptom and are not sure what is causing it
  • β€’You need an initial assessment before deciding on the right specialist
  • β€’Your concern spans multiple body systems or could be caused by many things
  • β€’You need a prescription renewal or chronic condition review
  • β€’You want a general health check-up or preventive health advice
  • β€’You need a referral letter or medical certificate
  • β€’The symptom is common and likely to be managed in primary care (colds, UTIs, mild rashes)
  • β€’You are uncertain and want to be guided to the right specialist efficiently

When to book a specialist directly

Book a specialist directly when:

  • β€’You already have a diagnosis and need specialist management (e.g., you know you have atrial fibrillation)
  • β€’You want a second opinion on a diagnosis or treatment plan
  • β€’Your GP has told you to see a specialist but you want faster access
  • β€’Your concern is clearly within one specialty area (heart symptoms β†’ cardiologist)
  • β€’You are seeking specialist-delivered psychological therapy
  • β€’Your skin condition has not responded to GP prescriptions
  • β€’You have a complex chronic condition that has outgrown primary care management
  • β€’You want pre-conception or fertility assessment

How referrals work on GeraClinic

If you book a GP and they recommend a specialist, they provide a referral letter with your clinical history, current medications, investigation results, and the specific clinical question for the specialist. You can book the specialist directly on GeraClinic using this referral β€” in many cases on the same day.

Referral letters are also valid for external clinics, NHS waiting lists, and insurance claims. They are stored in your GeraClinic health record and downloadable at any time.

Importantly, on GeraClinic, a referral is never required to book a specialist. It is a clinical document facilitating care continuity, not a gatekeeping mechanism.
